Sue’s story

When Sue left hospital after a fall at home, she needed more help to stay safe and live independently. Honeycomb Group’s Wellbeing Support Service helped Sue recover at home through tailored support.

After leaving hospital, many people need help to recover and stay safe during a challenging time. Honeycomb Group’s Wellbeing Support Service helps people bridge the gap, working alongside the Integrated Discharge Hub, Adult Social Care and Stoke-on-Trent City Council’s emergency duty team to support people leaving hospital to live independently at home.

When Sue left hospital, she was referred to Wellbeing Support Service because she lived alone and needed extra help during her recovery. Sue had no family living nearby and was struggling to cope at home with her mobility, accessing home safety equipment and cooking meals for herself.

After the referral was made, the Honeycomb Group’s wellbeing specialists visited Sue at home within 24 hours. They put a plan in place to complete urgent home adaptations to reduce the risk of further falls, emergency food support including the provision of hot meals, regular welfare checks and a referral to longer-term social care.

In just a few weeks Sue’s confidence grew, and she began preparing small meals again, could move around her home more safely, and even picked up her knitting needles again. Sue said, “I don’t know what I would have done without them. They made me feel like I wasn’t alone.”

Honeycomb’s team said, “It’s stories like this that remind us why our work matters. Not only did we help prevent another hospital admission, but we also helped restore a sense of dignity and independence to someone in a vulnerable moment.

By working closely with hospitals, local authorities, and emergency services, we ensure residents like Sue aren’t left to face challenges alone, and can remain safe and well in their home, their happy place.”

The Wellbeing Support Service helped Sue’s health and wellbeing, whilst also directly supported the NHS and Adult Social Care by preventing avoidable hospital readmission and reducing pressure on emergency services. Most importantly, Honeycomb Group’s work meant Sue could stay in the place she calls home.
